READ THIS INFORMATION CAREFULLY PRIOR TO COMPLETING YOUR FORM.
THE FORM CAN BE COMPLETED ON YOUR CELL PHONE OR COMPUTER. IF UNSURE WHICH ANIMAL YOU ARE BRINGING TO THE FAIR, THEN COMPLETE ON ENTRY DAY. POULTRY AND RABBITS MAY USE THEIR PEN NUMBER RATHER THAN TATTOO OR BAND NUMBERS FOR THE IDENTIFICATION.

Only complete one form for the animal(s) that you are exhibiting.

Make sure you know which animal(s) you plan to show before you complete the form.

Make sure to enter the correct tag number for your animal. Once you submit the form you will not be able to edit this information.

Need to list one contact cell phone number where you can be reached.

Everyone has completed Quality Assurance at this point. The answer to that question would be yes.

Medication only needs to be listed if the withdrawal time has not passed.

Read the medication question very carefully and answer correctly. If an animal is or was on medication, make sure to know the correct withdrawal information.

When you sign the form, you will need to use your mouse or touch pad. Parent signature is required for youth under 18 years old. Members 18 years and older may sign their own form.

Since their will be no lactating Dairy Cattle at the fair this year, Dairy Cattle do not need to complete a Drug Notification Form.
